Led and worked on a Healthcare Transformation consulting project for a primary care facility in Portugal.

This exhibited the importance of solid communication channels between care facilities and patients.

Purpose of Project:
To find the main reasons why elderly care is not efficient, and finding better communication channels between the primary care clinics and hospitals in Portugal
Background
Here are the 3 main issues that caused lack of communication between the hospitals and the primary care clinics
The hospitals and the primary care clinics are working in silos, which creates wait time for patients with chronic conditions.
Main Findings
Illness and treatment codes: The Primary care clinics and Hospitals use different coding systems which makes it difficult to communicate what the patient needs.
The current solutions are outdated and to not help the time efficiency issue.
